In order to expedite your request, it will be helpful if you could provide any of the following information: Case number, Date & Time, Location of Incident, the Name of the Deputy, or the Name of the person(s) involved.
Under Florida law, email addresses are public records. If you do not want your email address released in response to a public records request, do not send electronic mail to this entity. Instead, contact this office in person, by phone, or in writing.

If you wish to obtain a Traffic Crash Report Florida law requires that you complete the below form, indicating you are entitled to receive the report. Per Florida Statutes §316.066
Prior to 60 Days of Crash Report
Traffic crash reports are confidential and prohibited from public review for sixty (60) days from the date the report was filed. Therefore, an affidavit is required prior to release. Some of those entitled to receive the report prior to 60 days are the parties involved in the crash, their legal representatives, their licensed insurance agents, their insurers, or insurers to which they have applied for coverage. If you wish to obtain an accident report prior to 60 days from the date of the report, Florida law requires that you complete an Affidavit for Crash Report (Prior to 60 days) indicating you are entitled to receive a copy of the report.
After 60 Days from Date of Crash Report
Effective March 1, 2023, crash reports remain confidential after 60 days of crash. Crash reports are still available to those parties outlined above for reports made immediately available prior to 60 days. After 60 days, crash reports are also made available to a limited number of permissible uses as outlined in 18 U.S.C. 2725 (b) and (c). These permissible uses are included on the Affidavit for Crash Report Information (More than 60 days). If you qualify as a person for immediate release or as a permissible use, please complete the Affidavit for Crash Report (More than 60 days), provide requested information or documentation, and return the notarized form to us.
**We must receive a completed/notarized affidavit prior to our response, or visit the Records Department in person at 77151 Citizens Circle, Yulee, FL to sign the affidavit in person with a picture ID.
You can also request the report through the FL DHSMV crash portal.
Payments must be made by cash, money order, or VISA Credit Card.
Effective January 1, 2026, the fees associated with public records requests for special services will be increased. This increase is necessary to offset rising operational costs and to ensure the continued provision of high-quality services to the public. All public records requests submitted on or after the effective date will be subject to the revised fee schedule. We appreciate your understanding and continued support of our services.

Public records request that require special computer runs or extensive manual searching of records are subject to additional fees. A deposit may be required according to the nature of the request for extensive research. There will be a special service charge ($22.81 per hour) when the nature or volume of public records to be inspected is such as to require extensive use of information technology resources or extensive clerical or supervisory assistance, or both. You will be provided with a good faith estimate for the cost of research/duplication/redaction of protected information of the public record, and approval of the estimate must be provided prior to the fulfillment of the records request.
